This is a fully remote position, open to applicants in New Mexico.
β’ Oversee your schedule to perform in-home assessments throughout the New Mexico region utilizing your personal vehicle.
β’ Organize and arrange annual in-home assessments, which includes reaching out and following up with participants and their families, confirming appointments, and keeping an orderly calendar.
β’ Execute person-centered, face-to-face assessments within participants' homes, potentially involving adults with intellectual and developmental disabilities and/or their caregivers, using assessment tools to collect information on daily living skills, behaviors, and functional abilities.
β’ Navigate and utilize various technology platforms (such as online assessment tools, data systems, PDFs, Microsoft Office) to document findings, complete templates, upload assessments, and accurately close each case promptly.
β’ Ensure all tasks adhere to confidentiality and HIPAA regulations, including the protection of participant information and adherence to established policies and procedures for managing sensitive data.
β’ Communicate with your supervisor, program coordinator, and fellow assessors to report on your activities and discuss case details.
β’ Engage in training sessions and team meetings.
β’ Associate's degree in social work, psychology, or another field related to behavioral health sciences.
β’ 2 years of experience in social services, nursing, or other pertinent work fields.
β’ Valid, unrestricted driver's license, proof of auto insurance, and dependable transportation.
β’ Ability to successfully pass a Motor Vehicle Record check.
β’ Excellent oral and written communication and documentation abilities.
β’ Competence with computers and software applications, including the capacity to learn new systems.
β’ Comfort in conducting in-home visits with reliable personal transportation.
β’ Strong time management and organizational abilities.
β’ High level of dependability, accountability, and teamwork.
β’ Comfort working with individuals who have complex needs.
β’ Bilingual skills, particularly in Spanish.
β’ Experience working with adults with disabilities or individuals receiving support services.
β’ Background in healthcare or human services environments.
