Freelance Case Manager – Medical Rehabilitation

This is a fully remote position, open to applicants in Germany.

📋 Description

• Enhancing the evaluation of medical care requirements and the favorable functional outcomes for rehabilitation clients.

• Providing vocational counseling and job placement for rehabilitation clients, including profiling, application training, and job acquisition, all delivered through personalized one-on-one pedagogical support with appropriate written documentation.

• Serving as a person-centered "navigator" to ensure systematic coordination of care.

• Collaborating on a case basis within a network of payers and providers, including rehabilitation facilities, social benefit agencies, counseling centers, and self-help groups.

⛳️ Requirements

• A completed degree in social sciences (social work, education) or psychology, along with a minimum of 2 years of professional experience in vocational (re)orientation counseling.

• If holding a degree in psychology, you must also provide proof of at least 40 teaching units in social-law knowledge.

• A vocational qualification in health and social care or further training in rehabilitation, accompanied by at least 5 years of professional experience in vocational (re)orientation counseling, including evidence of methodological and social-law competencies (160 units of counseling competence and 40 units of social law).

• Preferred qualifications include further training in Case Management or CDMP certification.

• Familiarity with the structures of social benefit agencies and rehabilitation law.

• Experience in vocational and medical rehabilitation, as well as in solution-focused casework.

• Proficient knowledge of MS Office programs (particularly Word) and internet applications.

• Mobility (local to place of residence) is required, preferably with access to a personal vehicle.
