
Foster Care Case Manager

This is a fully remote position, open to applicants in Illinois.
• Collaborate with DCFS, youth, birth families, foster parents, and other designated supports to promote the achievement of permanency for youth in care.
• Meet with members of the child and family team as needed and work to identify any obstacles to permanency.
• Obtain supports and services to address identified challenges.
• Deliver case management services to children and families in foster care.
• Oversee and monitor placements for youth in care.
• Keep current and precise documentation of contact notes, case notes, and client records.
• Coordinate connections to suitable agency and community resources.
• Build a therapeutic alliance and engage families, caregivers, and other service providers in the treatment process.
• Prepare for and actively engage in all required meetings with DCFS.
• Attend and provide testimony in court hearings as needed.
• A Bachelor's Degree in Social Work or a related Human Services field.
• A minimum of two years of experience is required for specialized casework assignments.
• Previous experience in case management is preferred.
• A Child Welfare Employee License is required.
• Strong written and verbal communication skills.
• Proficient in active listening and clinical documentation.
• Ability to maintain accurate case records and adhere to agency, DCFS, and court documentation standards.
• Capability to establish and maintain effective working relationships with youth, birth families, foster parents, caregivers, and service providers.
• Proficient in Outlook, Word, electronic case management systems, and virtual communication tools.
