Field Care Manager – Physical Health

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Evaluate and assess the needs and requirements of members to achieve and/or maintain an optimal state of wellness.

• Assist members and their families in navigating and connecting with suitable resources for care and wellbeing.

• Perform both telephonic and in-person assessments to determine member needs.

• Regularly monitor member progress through ongoing assessments and evaluations.

• Create and execute personalized care plans based on clinical judgment.

• Collaborate with healthcare providers, community organizations, and services to ensure high-quality care.

• Document and submit incident reports in accordance with organizational policies and regulatory standards.

⛳️ Requirements

• Active Registered Nurse license in the state of Louisiana with a minimum of 2 years of experience.

• Experience working with diverse populations (youth, adults, geriatric).

• Familiarity with community health and social service agencies, as well as additional community resources.

• Proficient in using various electronic information applications/software programs, including electronic medical records, with strong keyboard and web navigation skills, and the ability to provide narrative documentation.

• Intermediate to advanced computer skills and familiarity with Microsoft Office.

• Capability to work independently within the role's scope and responsibilities.

• Must reside in Louisiana and be available for a full-time (minimum of 40 hours) flexible work schedule, including travel to members' residences within a 50-mile radius.
