
Field Care Manager, Behavioral Health

This is a fully remote position, open to applicants in Illinois.
• Conduct assessments in person and via phone with members.
• Act as the main contact, delivering integrated care.
• Utilize various strategies to address a member's health concerns.
• Carry out all necessary assessments, including the Comprehensive Risk Assessment.
• Coordinate both behavioral health and medical services.
• Enhance members’ health literacy and tackle social needs.
• Organize interdisciplinary care team meetings and facilitate communication among providers.
• Assist with transition of care initiatives through biannual and quarterly visits.
• Collaborate with internal teams, healthcare providers, and community organizations.
• Must be a resident of Illinois.
• Hold an active Illinois license as LCSW, LMFT, or LCPC (No supervisees or provisional licenses allowed).
• Have a minimum of 2 years of post-degree clinical experience in a behavioral health environment.
• Possess case management experience with complex populations including SMI, SUD, and SED.
• Must be able to travel to regional facilities and homes for in-person assessments.
• Have a valid driver's license, car insurance, and dependable transportation.
