• Start your day by reviewing your case load and preparing your technology.

• Utilize modern technology to navigate your daily schedule and collaborate with clinical in-home team members.

• Establish a warm and welcoming rapport with a diverse array of patients, gathering pertinent information and performing various services.

• Participate in proactive care by conducting comprehensive health risk assessments, medication reviews, and cognitive screenings.

• Collaborate directly with Sprinters to assess vital signs, schedule blood draws, and execute necessary tasks to meet the specific needs of each patient.

• Deliver thorough care coordination and management, encompassing preventive care interventions, medication oversight, referrals to specialists, and documentation of findings.

⛳️ Requirements

• Board Certified as a Family Nurse Practitioner.

• Active Family Nurse Practitioner License in one of the following states: TX, LA, OK.

• Consistently demonstrates the highest standards of professionalism, integrity, accountability, confidentiality, care, and compassion in delivering quality health services.

• Willingness to thrive in a groundbreaking environment that may require last-minute problem solving and creative thinking.

• Technologically proficient and comfortable using tools such as laptops or mobile devices for charting and HIPAA-compliant messaging apps for care coordination.

• Excellent written and verbal communication abilities.

• Capability to work independently or collaboratively in a team setting.

• Successfully pass a national background check and maintain a valid clinical license.
