Family Nurse Practitioner – Kentucky Licensed

This is a fully remote position, open to applicants in Kentucky.

📋 Description

• Assess the case load and prepare technology for patient services.

• Coordinate the daily schedule to collaborate with in-home clinical team members.

• Establish connections with a diverse array of patients while gathering pertinent information.

• Perform comprehensive health risk assessments, medication evaluations, and provide educational resources.

• Collaborate directly with Sprinters to assess vital signs and meet patient needs.

• Deliver thorough care coordination, including preventive care interventions and documentation.

⛳️ Requirements

• Must be Board Certified as a Family Nurse Practitioner.

• Possess an active Family Nurse Practitioner license in Louisiana.

• Consistently demonstrate the highest standards of professionalism, integrity, accountability, confidentiality, care, and compassion to deliver high-quality health services.

• Willing to work in an innovative environment that may require last-minute problem solving and creative thinking.

• Proficient with technology and comfortable using devices such as laptops or mobile phones for documentation and HIPAA-compliant messaging applications for care coordination.

• Excellent written and verbal communication skills.

• Capable of working independently or as part of a team.

• Must pass a national background check and have a valid clinical license verification.
