Enhanced Case Management Pharmacist

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Perform thorough clinical evaluations to determine member needs and opportunities for enhanced outcomes

• Work in collaboration with providers and care teams to create tailored care plans

• Track treatment advancement and intervene when care gaps or compliance issues are detected

• Build trusting relationships with members through proactive communication

• Tackle obstacles to treatment adherence

• Collaborate with internal teams to improve member experience and health outcomes

⛳️ Requirements

• Doctor of Pharmacy (PharmD) is mandatory

• A valid pharmacy practice license is required

• At least 3 years of experience as a licensed pharmacist in a clinical or patient-facing capacity

• Preferred additional certifications in diabetes care and patient education
