ECM Clinical Manager, LCSW/LMFT

This is a fully remote position, open to applicants in Arizona, +10 more states.

📋 Description

• Supervise a team of around 10-15 direct reports within the ECM team to guarantee high-quality enrollments and care management services. *The precise number of direct reports may vary based on business requirements.*

• Manage client outreach, enrollment processes, and the development of Individual Care Plans, evaluating whether clients' needs can be addressed by an ECM Lead Care Manager; act as an escalation point when necessary.

• Motivate the team to effectively achieve key performance indicators (KPIs).

• Partner with the Director(s) of Enhanced Care Management to facilitate changes and enhance processes.

• Provide clinical oversight for approximately 30% of the time, including, but not limited to: supervising clinical staff, clinical sign-offs, overseeing care plans, leading IDT meetings, and acting as a clinical consultant for complex clinical cases.

• Offer direct care management as required to support organizational objectives.

• Travel up to 20-25% may be necessary for training, meetings, and other business requirements.

⛳️ Requirements

• An active, unrestricted LCSW or LMFT license in California is required.

• Willingness to obtain additional state licensure as needed.

• Minimum of 2 years of experience in people management. *4 years preferred.*

• 2-3 years of experience in care management.

• 1-2 years of experience in post-acute care.

• A proven, results-driven leader with experience in change management within rapidly growing environments.

• Strong organizational skills and ability to work independently.

• Proficiency in Microsoft and Google applications; experience with Salesforce is an advantage.

• Exceptional communication and motivational abilities, including conflict resolution skills.

• Familiarity with lean Six Sigma methodology is* preferred.*
