This is a fully remote position, open to applicants in Michigan.
β’ Collaborate with clients to create a service delivery plan tailored to their identified needs, available community resources, and financial situation.
β’ Guide clients to necessary services while coordinating and overseeing the access to and provision of high-quality services.
β’ Conduct weekly monitoring and coordination through communication with clients and community resource service providers to ensure optimal use of available services and adequate delivery of required services.
β’ Maintain and update comprehensive client records, including demographic information, proof of status, care plan implementation, and care evaluation.
β’ Provide education on risk reduction (such as safer sex practices and safer injection techniques) to prevent comorbidity risks and reduce community viral load.
β’ Assist in creating and sustaining a network referral system within the healthcare and social service sectors by familiarizing yourself with both formal and informal services available to clients, reporting on new services or changes to existing services, and identifying gaps in community service availability.
β’ Link clients to resources to address mental health, substance abuse, and other psychosocial needs.
β’ Advocate for clients when necessary.
β’ Carry out outreach and case-finding initiatives as required to ensure clients remain engaged in care.
β’ Enter and maintain client-level data in the specified data system.
β’ Attend bi-weekly multi-disciplinary case conference meetings.
β’ An Associate degree or a High school diploma (or equivalent) is required, accompanied by at least one year of experience in human services.
β’ Ability to operate independently, demonstrate self-motivation, effectively engage with individuals from diverse socioeconomic backgrounds, and maintain high levels of organization.
β’ Familiarity with community resources is advantageous.
β’ Access to reliable transportation is necessary.
β’ Basic computer skills are required, especially in Microsoft Word and Excel.
