This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Act as a key contact for Hospital/ED inquiries to assist in discharge planning and coordination of care requirements after an ED/inpatient admission.

• Provide short-term care coordination on an individual basis, which may involve collaborating with management to eliminate obstacles that expedite ED discharges; for inpatient members, ensure safe and timely discharges by offering viable and appropriate recommendations.

• Connect and/or refer members, guardians, hospital/ED staff, and stakeholders to various services, providers, or community resources tailored to the specific population served.

• Engage in various cross-functional projects or tasks.

• Review and manage data, information, and documentation effectively.

• Collaborate with internal departments to identify, communicate, and track necessary services, providers, and resources.


⛳️ Requirements

• High School Diploma/GED with six (6) years of experience in Adult and Child behavioral health/mental health, developmental disabilities, or substance use populations.

• OR an Associate's degree with four (4) years of experience in Adult and Child behavioral health/mental health, developmental disabilities, or substance use populations.

• OR a Bachelor’s degree with a minimum of two (2) years of experience in Adult and Child behavioral health/mental health, developmental disabilities, or substance use populations.

• OR an equivalent combination of education and experience.

• Must possess a valid driver's license.

• Must be a resident of North Carolina.

• Must be able to travel within the designated catchment area as required.

• Preferred: Experience in providing crisis intervention or crisis response to individuals within the behavioral health/mental health, developmental disabilities, or substance abuse populations.

• Must be a fluent Spanish speaker.
