Directing Registered Nurse, eHomecare, Part-time

This is a fully remote position, open to applicants in Canada.

📋 Description

• Conduct remote assessments and manage care services for clients in their residences.

• Offer clinical guidance and delegate tasks within the unregulated care provider's (PSW/HCT) practice framework, fostering a collaborative approach to patient care.

• Keep precise clinical records for clients in both digital and paper formats.

• Manage patient assignments within the virtual ward effectively.

• Collaborate with all team members to ensure that all patient care needs are addressed.

• Utilize effective communication during daily huddles and while responding to emails and phone calls.

• Engage with external service provider agencies and Case Managers as needed.

• Contribute as a valued member of the multi-disciplinary healthcare team.

• Provide clinical leadership, guidance, and mentorship to the healthcare team.

• Foster and facilitate a collaborative team-based atmosphere.

• Build and sustain relationships with local healthcare providers.

• Participate in CNO Reflective Practice.

• Adhere to VON standards and specific eHomecare processes.

• Show readiness to engage in internal projects and continuous improvement initiatives.

• Exhibit professionalism within a diverse work environment.

• Demonstrate adaptability in managing changes within a fast-paced virtual setting.

⛳️ Requirements

• Must be registered and in good standing with the College of Nurses of Ontario.

• DRNs are required to hold a CNA Certificate in Palliative Care or have completed both the “Fundamentals of Palliative Care” and the “Comprehensive Advanced Palliative Care Education (CAPCE).”

• Pediatric DRNs should possess extensive hands-on experience in providing Pediatric Care.

• Recent experience in palliative, pediatric, and/or acute medical care for COPD and CHF patients is required.

• Proven advanced assessment skills are a must.

• Experience in gerontology and/or medical-surgical settings is beneficial.

• Background in a community-based healthcare system is an advantage.

• Demonstrated competencies in observation, independent assessment, health education, promotion, and familiarity with COPD/CHF clinical pathways.

• Experience with digital and electronic health systems, including EMR and mobile technology.

• Proficient nursing skills with the capability to interpret electronic technical information related to client outcomes and provide clinical direction.

• Critical thinking, problem-solving, and multi-tasking abilities are essential.

• Ability to function in a multidisciplinary virtual team environment while also working independently is necessary.

• High-speed internet access, computer skills, and a conducive home office setup are required.

• Candidates must be eligible to obtain a security screening certificate from the Government of Canada successfully.

• A valid CPR C / First Aid certification is required.

• Access to a personal vehicle for business purposes, along with a current driver’s license and proof of vehicle insurance, is mandatory.

• A current and original satisfactory Criminal Records Check is necessary prior to the start of employment.

• A Vulnerable Sector Search and/or Child Abuse Registry Check may be needed.

• Proficiency in French is considered an asset in designated French areas.

• Must be able to wear Personal Protective Equipment (PPE).
