This is a fully remote position, open to applicants in Texas.
• Offering support and motivation to patients and families undergoing or who have recently faced a behavioral health crisis.
• Manage case referrals, evaluate resource requirements and care gaps, review the progress of cases, and decide on case closure.
• Aid patients and families in achieving their personal recovery objectives through psychoeducation and connections to community resources.
• Evaluate social determinants of health and obstacles to care, including but not limited to housing, income, food insecurity, and transportation requirements.
• Link patients and families to support groups, community networks, and resources that promote mental wellness.
• Work collaboratively with interdisciplinary team members to advance the treatment plan objectives for patients and families.
• Oversee care management in coordination with agencies involved with the child, such as DCFS, educational institutions, external care team members, and others.
• Engage in regular consultations with the interdisciplinary team to track patient progress.
• Carry out additional responsibilities as assigned.
• Master’s degree in Social Work (MSW) is required.
• Bilingual proficiency in English and Spanish is preferred.
• A minimum of 2 years of case management or relevant experience is necessary.
• Experience in team-based care with strong collaboration skills.
• Familiarity with child and adolescent case management, school-based care, inpatient settings, and/or community mental health.
• Knowledge of acute behavioral health needs (suicidality, homicidality, severe persistent mental illness).
• Strong understanding of community resources.
