πŸ“‹ Description

β€’ Provide a supportive and positive experience for clients, referral sources, and external providers.

β€’ Engage directly with clients, families, and referral sources to comprehend their needs and preferences.

β€’ Make precise and timely referrals to aftercare resources for clients transitioning from Charlie Health.

β€’ Utilize and maintain a nationwide provider database to locate suitable referral options.

β€’ Facilitate referrals using the preferred communication channels of external providers.

β€’ Conduct follow-ups with clients and/or external providers to confirm placements.

β€’ Communicate with referral providers regarding new referrals, ensuring they have all the necessary information to evaluate a referral; assist in ensuring that all receiving providers possess the required clinical materials and information.

β€’ Document case management interactions in progress notes, and communicate with therapists, case managers, and the treatment team about contact and updates on discharge planning status.

β€’ Collaborate closely with internal stakeholders at Charlie Health (e.g., clinical team, admissions team) as needed to fulfill job responsibilities.

β€’ Work in partnership with the Clinical Outreach and Partnerships teams to gain a thorough understanding of referral sources and the services they offer.

β€’ Comply with established policies and procedures while achieving performance metrics goals.


⛳️ Requirements

β€’ Bachelor’s degree in health sciences, communications, psychology, social work, or a related field.

β€’ At least 2 years of relevant work experience (e.g., healthcare experience, preferably in customer/patient-facing roles such as case management, discharge planning, referral relations, admissions, or outreach).

β€’ Strong interpersonal, relationship-building, and listening abilities.

β€’ Metrics- and results-driven mindset, with experience working towards specific targets.

β€’ Exceptional written and verbal communication skills.

β€’ High level of organization and attention to detail.

β€’ Authorized to work in the United States with native or bilingual English proficiency.

β€’ Ability to excel in a fast-paced environment and adapt quickly.

β€’ Proficient in Salesforce and Google Suite/MS Office.
