This is a fully remote position, open to applicants in Australia.
β’ Provide thorough remote assessment, monitoring, and care coordination for individuals with chronic and complex health issues.
β’ Recognize and address early indicators of clinical decline, escalating care and organizing timely interventions as necessary.
β’ Work in partnership with consumers, caregivers, and a multidisciplinary healthcare team to deliver high-quality, person-centered care.
β’ Engage in quality improvement initiatives, enhance service outcomes, and pursue continuous professional development while participating in a 7-day roster.
β’ Active AHPRA registration as a Registered Nurse accompanied by a Bachelor of Nursing degree.
β’ Proven experience in managing chronic diseases, conducting comprehensive clinical assessments, and crafting person-centered care plans.
β’ Excellent communication, organizational, and clinical decision-making abilities, with a proven track record of working effectively within a multidisciplinary team.
β’ Background in community or home-based healthcare environments, possessing strong computer skills and a valid driver's license.
