Connected Care – Remote Patient Monitoring

This is a fully remote position, open to applicants in Australia.

📋 Description

• Provide thorough remote assessment, monitoring, and care coordination for individuals with chronic and complex health issues.

• Recognize and address early indicators of clinical decline, escalating care and organizing timely interventions as necessary.

• Work in partnership with consumers, caregivers, and a multidisciplinary healthcare team to deliver high-quality, person-centered care.

• Engage in quality improvement initiatives, enhance service outcomes, and pursue continuous professional development while participating in a 7-day roster.

⛳️ Requirements

• Active AHPRA registration as a Registered Nurse accompanied by a Bachelor of Nursing degree.

• Proven experience in managing chronic diseases, conducting comprehensive clinical assessments, and crafting person-centered care plans.

• Excellent communication, organizational, and clinical decision-making abilities, with a proven track record of working effectively within a multidisciplinary team.

• Background in community or home-based healthcare environments, possessing strong computer skills and a valid driver's license.
