This is a fully remote position, open to applicants in United States.
• Prioritize and oversee individual members throughout their journey with Twin, focusing on reversal and self-care management.
• Provide remote coaching using applications, video conferencing, and the Twin Health electronic system.
• Engage and empower members to set personal goals, manage their health, and maintain motivation on their path to self-efficacy.
• Inspire, coach, and deliver outstanding service and support to assist members in achieving their health objectives as outlined by the Twin app.
• Conduct thorough daily monitoring and management of assigned members to ensure full compliance with the Twin program.
• Review member health data and reports on a daily basis to ensure accuracy, completeness, and adherence to the program.
• Provide feedback to members regarding daily metrics such as weight, continuous blood glucose monitoring, blood pressure, sleep, breathing, movement, dietary and supplement adherence, and overall satisfaction with the program.
• Rigorously follow Standard Operating Procedures for member management and escalations.
• Collaborate closely with colleagues in the coaching team, providers, registered nurses, and operations teams to create a seamless experience for members.
• Work with a multidisciplinary team within Twin and the academic environment to address patient care issues and help members achieve chronic disease prevention and reversal.
• Organize and maintain participant records securely and in compliance with regulations.
• Perform additional duties as assigned.
• A minimum of 3 years of relevant experience, potentially in Health/Nutrition counseling, health coaching, fitness coaching, life coaching, or similar fields.
• Active license in the US; Health Coaching or Health Educator certifications are preferred.
• Bilingual with fluency in both English and Spanish.
• Proficient in motivational interviewing and fostering behavior change.
• Comfortable and enthusiastic about adopting the latest technologies, incorporating data and technical outputs into patient care.
• Ability to navigate ambiguity and change effectively.
• Experience in a high-growth or rapidly changing environment.
