Complex Case Manager, RN

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Oversee a designated panel of members by conducting continuous evaluations of their health management requirements.

• Develop care plans tailored to address the specific needs of members.

• Achieve the targeted percentage of patient goals.

• Ensure that all activities are properly documented and adhere to relevant compliance requirements.

⛳️ Requirements

• A High School Diploma or GED is mandatory.

• A minimum of 7 years of experience in clinical, case management, and/or disease or condition management roles is required.

• Must possess a current RN licensure in the State of Pennsylvania OR hold a current multi-state license through the enhanced Nurse Licensure Compact (eNLC), or have licensure in West Virginia, Delaware, or New York.

• Certification in Case Management is preferred.
