Community Wellness Clinical Program Manager

This is a fully remote position, open to applicants in Alaska.

📋 Description

• Coordinate the provision of grant-funded, clinically integrated primary and secondary prevention services.

• Supervise the integration of screening, patient education, and lifestyle counseling within the outpatient healthcare framework.

• Manage grant expenditures and suggest strategies for fiscal responsibility across federal, state, and/or tribal funding sources.

• Oversee patient screening, education, and the entry of follow-up data into Cerner and other registries/tracking systems.

• Conduct quality assurance evaluations in collaboration with Community Wellness staff, clinic managers, and members of the medical executive committee.

• Provide oversight for staff engaged in health education and case management for patients who are due for screening and diagnostic services.

• Supervise patient tracking and follow-up, ensuring high-quality delivery of treatment services, lifestyle interventions, and further follow-up.

• Collaborate with SEARHC departments and strategic partners to enhance access to comprehensive prevention and healthcare services.

• Coordinate program activities, service documentation, eligibility screening, grant-funded reimbursements, and service expansion initiatives.

• Work collaboratively with local, state, and federal agencies.

• Partner with the Director of Community Wellness, Grant Writers, and the Finance team to prepare annual reports, continuation and competitive applications, and service indicator reporting.

• Hire, train, evaluate, and manage subordinate staff.


⛳️ Requirements

• A Master of Public Health or another professional health degree; a Bachelor’s degree combined with 2 additional years of relevant experience may substitute for a Master’s degree.

• A minimum of 3 years of experience in healthcare, public health, nutrition, or related fields, including grant coordination or management, health education, motivational interviewing, or lifestyle counseling in a public health or ambulatory care context.

• Experience navigating complex patient care and referral systems.

• Certification in Basic Life Support.

• At least 2 years of supervisory experience.

• Advanced knowledge of cardiovascular health, breast, cervical, and colorectal cancer prevention and screening, diabetes prevention and management, tobacco control and cessation, physical activity promotion, and nutrition.

• Familiarity with health education core competencies, stages of change theory, motivational interviewing, and the socioecological model.

• Proficient understanding of group dynamics with the ability to facilitate and train groups.

• Strong customer service skills.

• Advanced computer skills, including email, word processing, spreadsheets, databases, and document organization.

• Exceptional oral and written communication skills, with the ability to relate to patients and staff in a sensitive and culturally competent manner.

• Outstanding organizational and prioritization abilities.

• Problem analysis and resolution skills in complex healthcare environments using a systems approach.

• Preferred proficiency in SEARHC’s electronic health data system.

• Capability to provide guidance and leadership to clinic staff while integrating service processes into existing clinic protocols.

• Ability to collaborate with other programs to maximize service delivery and minimize service duplication.

• Willingness to travel to grantor and partner meetings, as well as site and supervisory visits.
