Community Health Worker – Field MI, Wayne and Macomb Counties

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Act as a liaison between community members and the healthcare system by engaging in outreach and educational efforts.

• Offer medical information to healthcare professionals and care coordinators via telephone communications.

• Have a comprehensive understanding of both formal and informal community agencies and resources.

• Possess working knowledge of multi-system outreach initiatives related to healthcare delivery, clinical education, and health-related services.

• Advocate for adjustments in medical interventions by influencing providers to consider members’ cultural, ethnic, or religious preferences.

• Collaborate effectively within a team, manage various priorities, employ strong time management skills, and demonstrate sound judgment.

⛳️ Requirements

• Must reside in Wayne County, Macomb County, or the nearby service area.

• Willingness and ability to travel within a 90-mile radius up to 70% of the time is essential.

• Reliable transportation is required, along with the ability to travel throughout the designated territory.

• A minimum of 1-3 years of relevant experience is necessary, ideally in community outreach, member engagement, patient support, care coordination, or conducting home visits.

• Experience with diverse urban and/or rural populations is preferred, along with a strong grasp of local community resources, support networks, and service organizations.
