Community Health Worker, Certified

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Engages in patient-centered outreach primarily through telephonic communication with individuals facing identified social needs.

• Works collaboratively with healthcare teams, community-based organizations (CBOs), and colleagues at the hub to evaluate needs and coordinate suitable support.

• Applies motivational interviewing and problem-solving strategies to educate patients and connect them with community resources that address social determinants of health.

• Performs follow-up outreach to verify successful connections and tackle any barriers, thereby supporting a shared team workflow and accountability model.

• Ensures accurate and timely documentation in Geisinger’s electronic medical record (EMR) to capture outreach efforts, referrals, and outcomes.

⛳️ Requirements

• High School Diploma or Equivalent (GED) - (Required)

• A minimum of 1 year of relevant work experience (Required)

• Certified Community Health Worker (CCHW) - Certification from the Pennsylvania Certification Board
