Community Health Worker

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Serves as a link between healthcare providers, community-based organizations, and individuals within the community.

• Recognizes the experiences, socioeconomic needs, language, and cultural aspects of the communities served.

• Engages in outreach, delivers health education, and assists patients in navigating health systems.

• Identifies obstacles to care and aids in modifying service delivery to enhance patient and community health.

• Fosters individual and community capacity by enhancing health knowledge and self-sufficiency through outreach, community education, social support, and advocacy.

• Acts as a liaison between communities and healthcare systems/agencies.

• Provides guidance and social support to patients and the entire care team.

• Improves patients' ability to communicate effectively with healthcare providers and care team members.

• Delivers culturally and linguistically appropriate health education utilizing evidence-based resources.

• Advocates for patients, families, caregivers, and communities to secure necessary care and/or resources.

• Updates healthcare providers and care team members about barriers and challenges affecting patients' capabilities.

• Establishes and nurtures collaborative relationships with community-based organizations and across the care continuum.

⛳️ Requirements

• High School Diploma/GED.

• One year of knowledge regarding local community and resources.

• Minimum of one year of experience in a community-based environment.

• Capable of functioning both independently and collaboratively.

• Proven ability to initiate, accept, and adapt to change.

• Demonstrated time management and organizational skills.

• Accepts and acknowledges diversity among individuals.

• Basic computer proficiency.

• Ability to engage with diverse populations.

• PA State Certified Community Health Worker - State of Pennsylvania within 1 year.

• American Heart Association Basic Life Support certification - State of Pennsylvania upon hire.

• Valid Pennsylvania Driver's License upon hire.

• Bilingual in English/Spanish (written & oral) preferred.
