Community Health Worker

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Engages with the community to provide outreach to patients facing complex medical and social challenges.

• Serves as a valuable resource to help patients navigate the healthcare system effectively.

• Orients and evaluates new Community Health Workers.

• Regularly assesses the proficiency levels of staff during their orientation to support decisions regarding their training progression or necessary refocusing until proficiency is achieved.

• Aids in identifying and reaching out to medically complex patients.

• Collaborates with the multidisciplinary team to develop and implement comprehensive care plans.

• Engages patients through various means including phone calls, home visits, hospital visits, and accompanying them to appointments with providers as needed.

• Completes vital sign assessments.

• Identifies care gaps and connects members with appropriate resources.

• Facilitates telemedicine appointments between members and providers.

• Provides disease-specific education.

• Conducts medication reviews to ensure adherence to prescribed regimens.

• Performs safety evaluations in patients' homes.

• Promotes adherence to treatment plans by proactively identifying and resolving gaps in care.

• Communicates concerns to the clinical team and manager as necessary, seeking support and guidance in a proactive manner.

• Participates in multidisciplinary team conferences to optimize treatment plans.

• Reinforces the treatment plan under the direction of the Case Manager and Licensed Social Worker, and is available for consultation and case review when needed.

• High School Diploma or Equivalent (GED) - (Required)

• At least 1 year of relevant work experience - (Required)

• Basic Life Support Certification - Default Issuing Body

• Valid Driver's License - Default Issuing Body
