Community Engagement Specialist

This is a fully remote position, open to applicants in California.

📋 Description

• Perform in-person outreach efforts for members identified through health plan eligibility lists.

• Complete documentation in Athena for each outreach effort.

• Schedule intake appointments and aid in filling out necessary forms.

• Participate in outreach events, professionally representing Zócalo Health to community members and partners.

• Establish and nurture relationships with community coalitions, CPI collaboratives, and other stakeholders to maintain their engagement.

• Convert leads into patients using internal documentation systems.

• Record all outreach attempts and results in Athena and the relevant trackers.

• Assist in dispatching patient mailers and making follow-up calls to leads generated from community events to enroll them in programs.

• Represent Zócalo Health at community coalition meetings and collaborative gatherings.

• Ensure consistent, professional messaging while building trust with community stakeholders.

⛳️ Requirements

• A minimum of 1-3 years of experience in healthcare, community outreach, or a similar field.

• Proven experience in engaging with diverse communities and providing culturally sensitive communication.

• Proficiency in both English and Spanish (verbal and written).

• Strong interpersonal and communication skills to foster trust and connect with patients and community partners.

• Detail-oriented with excellent organizational abilities.

• Comfortable with technology and skilled in using electronic health records (preferably experience with Athena).

• Capability to work a flexible schedule, including evenings and weekends, to attend events.

• Access to reliable transportation for in-person outreach and events.
