Co-Lead, ROOTS Hub Initiative

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Assist with the execution and operational management of Initiative 1 within the designated region 3 of NCDHHS.

• Offer leadership and coordination among healthcare providers, community organizations, local health departments, behavioral health agencies, social service organizations, and regional implementation partners.

• Work in partnership with the ROOTS Hub Implementation Manager and region 3 lead anchors to customize NCRHTP initiatives and activities.

• Monitor Vaya’s RHTP plan to ensure compliance and performance in relation to program deadlines and milestones.

• Promote collaboration between providers, community organizations, and regional partners.

⛳️ Requirements

• A Bachelor’s degree in public health, healthcare administration, social work, business administration, public administration, or a related field is required.

• A minimum of two (2) years of experience in managing large and complex projects.

• At least two (2) years of experience in coordinating efforts among diverse stakeholders.

• A minimum of two (2) years of experience in a setting that demonstrates knowledge of healthcare systems, Medicaid and/or Medicare payer systems, and the clinical and social needs affecting communities’ and/or populations’ access to health services.

• Excellent project management and organizational abilities.

• Proficient in Microsoft Office applications with the capacity to learn additional software systems.
