This is a fully remote position, open to applicants in United States.
• Developing, implementing, and supporting innovative workflows and processes—both clinical and technological—for clients and Sound physician advisors as part of Sound ASSURE.
• Regularly assessing processes, tools (including Sound Connect, Advisor Connect, and client EMRs), and workflows to pinpoint areas for enhancement.
• Acting as a primary contact for physician advisors and clients.
• Prioritizing case review requests to determine which cases require secondary review by physician advisors.
• Independently managing EMR accesses and notifying relevant team members for assistance when challenges arise.
• Generating PDF extracts of essential EMR components necessary for physician advisor case evaluations.
• Overseeing and prioritizing case requests to ensure adherence to target completion deadlines.
• Facilitating and executing workflows for all potential opportunities, particularly related to observation and short stay patients.
• Analyzing hospitalist patient lists for observation and short stay cases.
• Recording all steps in internal software.
• Completing additional special projects as assigned.
• Associate's or Bachelor's Degree in nursing or LPN certification preferred, or a related field.
• Over 3 years of experience in case management within an emergency or acute care environment.
• Strong clinical expertise and experience in the Acute Care Hospital setting (Critical Care, Emergency Department, and/or Case Management experience is beneficial).
• Capability to evaluate patient clinical status for admissions, observation, or discharge.
• Proficiency with electronic medical record systems.
