This is a fully remote position, open to applicants in California.
• The ALF Case Manager is responsible for coordinating the care of clients enrolled in the ALWP under the Care Coordination Agency (CCA).
• Care coordination involves the identification, organization, coordination, and monitoring of the needs of an ALW client.
• The ALF Case Manager verifies ongoing eligibility for the ALWP by performing monthly and as-needed follow-up visits for compliance and skilled needs.
• Verify Medi-Cal eligibility for each client on a monthly basis.
• Arrange monthly appointments with clients.
• Communicate with clients to assess if they require anything, such as Durable Medical Equipment (DMEs) or appointments.
• Inquire about medication adherence and other general assessment inquiries.
• Engage with staff to learn about any Serious Incident Reports (SIRs). If present, ensure that our office receives them.
• Identify if any individuals are absent and lacking an SIR. If so, inform the staff that they must obtain an SIR as soon as possible.
• Ensure to follow up with facility staff on this matter and confirm our office obtains a copy of the SIR.
• If there are Medi-Cal issues, communicate these to the client and facility staff, guiding them on how to resolve them if they are unaware.
• Consult with staff regarding medications and review the medication log.
• Ensure to update client information in the company tracking system regarding visit dates and when notes are uploaded in Devero.
• All documentation must be uploaded by the end of the next business day following the client visit.
• Ensure to document in your notes that you inquired about medication compliance from both the client and staff.
• According to the workflow process, when a client exits a facility, the CCA holds specific tracking responsibilities.
• As an ALF Case Manager, you are required to monitor when clients leave during your facility visits.
• Notify the ALF Coordinator when a client departs from and returns to the facility.
• During a Re-Assessment, the RN will inform you via email that it has been completed, and you will need to make a copy of the ISP for the client to sign.
• Provide a copy to the facility, then upload it to Devero and email your CCA to confirm completion.
• Ensure the client signs the document within 7 business days.
• Have knowledge of confidentiality, HIPAA, and healthcare laws and regulations.
• Maintain accurate timekeeping.
• Adhere to policies and procedures as outlined in the Employee Handbook.
• Keep all credentials current.
• Report instances of fraud and abuse.
• Have familiarity with mandated reporting.
• Conduct timely recording and/or documentation of client interactions.
• Attend Department meetings in person, by phone, or via web conferencing.
• Participate in all state-mandated in-service training sessions.
• Driving may be necessary to reach geographical areas covered by the company.
• A significant amount of driving may be required throughout Los Angeles, Fresno, San Diego, Riverside, and San Bernardino Counties.
• Perform additional duties as assigned.
• Bachelor’s Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology, along with one year of related work experience.
• Current CPR certification is required.
• Familiarity with The State of California Assisted Living Waiver Care Coordination Agency is preferred.
• Experience in education, training, quality assurance, and management is preferred.
• Demonstrated excellent judgment and knowledge of current clinical practices, with notable clinical, analytical, and documentation skills.
• Strong organizational abilities.
• Self-directed with the capacity to work independently with minimal supervision.
• Proficient in using computers and electronic information systems.
• Ability to establish and maintain effective communication and relationships with office, field, and administrative personnel.
• Familiarity with Title 22 is preferred.
• Must possess a valid driver’s license and have access to an insured automobile in good working condition, in compliance with state and/or organizational requirements.
