This is a fully remote position, open to applicants in Colorado.
• Plays an essential role in enhancing and supporting DaVita's Integrated Kidney Care (IKC) initiatives focused on improving Stars and quality metrics.
• Concentrates on advancing clinical quality, addressing care gaps, and promoting robust collaboration with payor partners to elevate the overall patient experience and the success of the programs.
• Utilize data and analytics to track clinical performance, develop and implement targeted outreach campaigns, and establish enduring reconciliation processes.
• Design, implement, and manage targeted campaigns aimed at closing care gaps.
• Collaborate with both clinical and non-clinical team members to uncover opportunities for patient outreach and clinical interventions.
• Oversee clinical and operational performance across assigned care pods and IKC markets, regularly presenting findings, trends, and actionable insights in workgroups and leadership meetings.
• Develop and maintain programs to facilitate reconciliation with payor partners.
• Ensure precise tracking, reporting, and alignment of clinical data with the requirements of external partners.
• Lead pilot projects to innovate our care model.
• Aid in the creation of training materials for frontline team members regarding care gap workflows.
