Clinical Pharmacy Specialist, HIV

This is a fully remote position, open to applicants in Oregon.

📋 Description

• Deliver thorough medication management services, encompassing reconciliation, dose optimization, adherence assistance, and drug-level evaluations in accordance with evidence-based practices.

• Collaborate with physicians, specialists, and interdisciplinary teams to create personalized care plans under a Collaborative Practice Agreement (CPA) and facilitate effective medication coordination.

• Assess the effectiveness and safety of therapies, recognize opportunities for optimization, and take appropriate action as needed.

• Contribute to quality improvement and Risk Evaluation and Mitigation Strategies (REMS) initiatives.

• Offer operational recommendations by assisting colleagues with complex prior authorizations, enhancing workflows, educating patients, and supporting ongoing clinical development.

⛳️ Requirements

• Possess a Doctor of Pharmacy (PharmD) degree.

• Must be a licensed pharmacist in the state of Oregon.

• Hold certification as an HIV Specialist (AAHVIP).

• Completed a PGY2 Ambulatory Care Residency or a PGY1 Residency along with a minimum of 1 year of ambulatory practice under a CPA, or have at least 3 years of ambulatory practice with disease state management.

• Demonstrate strong communication, organizational, and technological skills.
