Clinical Pharmacist – Care, Medication Therapy Management

This is a fully remote position, open to applicants in Kentucky.

📋 Description

• Oversee patient populations categorized as high risk to enhance disease state outcomes and ensure medication-related safety.

• Implement safe, effective, and economical medication therapies for all patients under care, while monitoring their treatment appropriately.

• Assist the quality department in enhancing HEDIS metrics through initiatives focused on medication adherence and regular health visits.

• Offer clinical recommendations on various topics, including gaps in care, high-risk medications, compliance and adherence, drug interactions, therapeutic substitution, and the use of generic alternatives.

• Leverage client formulary information and member-specific benefits to inform suitable medication recommendations.

• Stay informed about current and emerging clinical trends, providing education to the clinical team, physicians, and other prescribers.

• Engage in the creation of educational materials intended for presentation in educational intervention programs.

• Collaborate with physicians and other prescribers by reviewing patient drug profiles to prevent unintended side effects from new prescriptions and to ensure prescribed medications are eligible for coverage, as well as being cost-effective and high quality.

• Reach out to physicians to clarify medication regimens and offer recommendations.

• Conduct research and analysis to uncover opportunities for cost management and process or quality improvements.

⛳️ Requirements

• Bachelor’s degree or advanced degree (Pharm.D., M.S.) in pharmacy.

• A minimum of 2 years of experience in mail order, retail, hospital, managed care pharmacy, or completed a pharmacy residency program.
