This is a fully remote position, open to applicants in New Jersey.
β’ Perform thorough medication reviews and reconciliation services.
β’ Identify, evaluate, and resolve issues related to medications.
β’ Offer telephonic and virtual consultations to patients.
β’ Assist patients managing chronic conditions such as diabetes, hypertension, heart failure, COPD, and other complex health issues.
β’ Create personalized medication management recommendations for both providers and patients.
β’ Encourage medication adherence through patient coaching and motivational interviewing techniques.
β’ Engage in care transitions and conduct post-discharge medication review activities.
β’ Accurately document clinical interactions in line with regulatory and organizational requirements.
β’ Collaborate with physicians, nurse practitioners, and care teams to enhance patient outcomes.
β’ Participate in quality improvement initiatives and efforts to increase patient engagement.
β’ Doctor of Pharmacy (PharmD) degree from an accredited institution.
β’ Active New Jersey Pharmacist License in good standing.
β’ At least two (2) years of experience as a pharmacist in ambulatory care, clinical pharmacy, managed care, telehealth, or similar environments.
β’ Proficient in using computers and navigating various clinical systems.
β’ Ability to work independently in a remote setting while upholding productivity and quality standards.
β’ Willingness to acquire additional pharmacist licenses as business needs change.
β’ Capability to educate patients in the Spanish language.
