This is a fully remote position, open to applicants in Florida.
• Perform medication therapy evaluations and targeted outreach for routine clinical pharmacy programs following established protocols.
• Analyze medication-related quality care deficiencies, including adherence and statin-use opportunities, while supporting patient or provider follow-ups.
• Evaluate prescription histories, medication lists, refill behaviors, and basic drug utilization data to pinpoint issues needing intervention or escalation.
• Work in partnership with providers, care managers, pharmacy technicians, and other members of the care team to address adherence, safety, affordability, and access concerns.
• Provide education to patients and caregivers regarding medication use, barriers to adherence, available affordability resources, and pharmacist-approved care plan actions within the defined scope.
• Record clinical assessments, outreach outcomes, recommendations, and follow-up measures in relevant systems.
• Review medication histories and outreach documentation collected by pharmacy technicians for accuracy and clinical significance.
• Conduct thorough utilization management reviews for Medicare Part B-covered medical drugs, assessing medical necessity, FDA labeling, compendia-supported indications, prior authorization, step therapy, and site-of-care optimization.
• Collaborate with medical directors, providers, and care management teams on utilization trends for Part B and Part D, denial and appeal procedures, and exception requests.
• Utilize evidence-based criteria and Medicare coverage determinations to ensure appropriate utilization, support Stars/HEDIS measures, encourage biosimilar adoption, and facilitate lower-cost infusion transitions.
• Engage in an interdisciplinary Model of Care for Medicare Special Needs Populations.
• Doctor of Pharmacy degree or another pharmacy degree from an ACPE-accredited College of Pharmacy is required.
• An active, unrestricted pharmacist license in the applicable state of practice is necessary.
• Collaborative practice certification or a similar advanced credential may be required depending on the patient population and business needs.
• Additional state pharmacist licenses may be necessary based on patient population and business demands, or a willingness to obtain them as needed.
• Completion of an accredited pharmacy training program is mandatory.
• Experience in residency, managed care, ambulatory care, or value-based care is preferred.
• A foundational understanding of medication therapy management, drug utilization review, medication adherence, quality measures, patient counseling, and provider collaboration is essential.
