This is a fully remote position, open to applicants in Arizona, +5 more states.
• Oversee a multidisciplinary team comprising Registered Nurses, Licensed Vocational Nurses, Social Workers, Chaplains, and Medical Assistants.
• Engage in strategic planning to influence the future trajectory of clinical care delivery.
• Lead and participate in significant initiatives aimed at enhancing clinical care, quality, and the introduction of new programs.
• Identify, address, and resolve clinical workflow challenges that adversely affect care quality or operational results.
• Ensure sufficient and effective patient care coverage.
• Support the attainment of KPIs assigned to the care delivery team and conduct annual performance evaluations.
• Collaborate with Providers and a diverse professional team to improve patient care, implement changes, and develop process enhancements.
• Provide leadership in conflict resolution, mentoring, skilled decision-making, time management, adaptability, and commitment.
• Deliver direct patient care as necessary to meet organizational objectives.
• Participate in the development and onboarding of newly hired clinical staff (RN, SW, Provider, MA).
• Ensure the team adheres to regulatory compliance (state license renewals, certification check-offs, etc.)
• Active, unrestricted RN license with the capability to secure California licensure within 6 months of employment.
• Minimum of 2 years in a people management role.
• At least 5 years of experience in a direct care delivery position as a Registered Nurse, with a minimum of 2 years in palliative care, hospice care, or complex care management.
• Strong grasp of cultural competency relevant to the target population.
• Proven track record as a results-oriented leader with experience in change management within rapidly growing environments.
• Excellent organizational skills and ability to work independently.
• Outstanding communication and motivational abilities, including conflict resolution strategies.
• Prior experience working remotely with proficiency in Microsoft and Google applications; experience with Salesforce is advantageous.
