This is a fully remote position, open to applicants in United States.
• Manage, assess, and independently evaluate escalated clinical clearance requests received from the CAT/CAA team.
• Utilize nursing clinical judgment to assess the appropriateness of admissions for complex emergency department (ED) and substance use disorder (SUD) cases.
• Analyze and interpret ED medical clearance laboratory results (including electrolytes, CBC, prealbumin, phosphorus, and liver function) and integrate these findings into clearance decisions.
• Review EKG findings pertinent to ED cases — such as bradycardia, QTc prolongation, and ST/T wave changes — considering nutritional and electrolyte status.
• Evaluate orthostatic vital signs and identify signs of hemodynamic instability.
• Provide timely clearance outcomes (clear for admission or refer to higher level of care) and communicate directly with the CAT teams.
• Act as a clinical liaison between the CAT team and medical leadership; prepare organized case summaries for physician-level escalations.
• Monitor and follow up on outstanding lab results; collaborate with clients and providers to minimize admission delays.
• Maintain the clinical clearance queue in Salesforce; ensure prompt review of all 'Admission Pending — Clinical Clearance' cases.
• Coordinate MAT-related clearances based on state and insurance eligibility criteria; escalate issues to medical leadership as appropriate.
• Identify process inefficiencies and suggest updates to decision trees, templates, and standard operating procedures (SOPs).
• Participate in CAT team training regarding clinical clearance expectations and escalation criteria.
• Current Registered Nurse (RN) license in good standing; unrestricted licensure is mandatory.
• At least 2–3 years of nursing experience; experience in behavioral health, medical-surgical, or eating disorder/SUD-related environments is highly preferred.
• Familiarity with ED medical stability criteria: %IBW, percent weight change, BMI thresholds, and risk for refeeding syndrome.
• Capability to review and interpret ED lab panels and recognize clinically significant flags, including electrolyte imbalances, CBC irregularities, nutritional markers, and liver function indicators.
• Comfort in reviewing EKGs for relevant findings in relation to electrolyte status and nutritional presentation.
• Solid understanding of SUD presentations, including withdrawal risk, medication-assisted treatment (MAT), and SUD level-of-care criteria.
• Strong grasp of level-of-care frameworks (IOP, PHP, residential, inpatient) and LOCUS criteria.
• Exceptional written communication skills; ability to document clinical reasoning clearly and succinctly.
• Proficient in or able to quickly learn Salesforce or similar CRM/admissions platforms.
• Familiarity with ASAM criteria is preferred; experience in virtual/telehealth behavioral health environments is advantageous.
• Bilingual (English/Spanish) is a plus.
• Authorization to work in the United States.
• This position is a 1099 independent contractor role. Hourly compensation is based on experience and credentials.
