Clinical Case Manager – Behavioral Health

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Perform thorough assessments and develop personalized care plans.

• Facilitate care coordination with healthcare providers and link members to community resources.

• Tackle complex behavioral and medical issues utilizing strong clinical judgment.

• Offer crisis intervention and provide follow-up support.

• Foster positive and cost-effective health outcomes.

⛳️ Requirements

• Master’s degree in Social Work or Counseling is required.

• Must possess an active, unrestricted independent clinical license (LCSW, LISW, LCPC, LPC, LP, LMFT, or LMHC).

• A minimum of 3 years of post-master’s clinical experience in settings such as hospital, outpatient, or equivalent.

• In-depth knowledge of mental health and substance use disorders is essential.

• Proficiency in Motivational Interviewing is expected.

• Capability to manage multiple tasks in a dynamic, technology-driven environment.

• Experience in crisis intervention is considered a plus.

• Background in managed care, utilization review, or case management is also a plus.
