This is a fully remote position, open to applicants in Ohio.

📋 Description

• Proactively engages with children and youth experiencing significant behavioral health challenges, along with their families and caregivers, for an assigned caseload (1:10 for ICC and 1:25 for MCC).

• Develops and maintains a comprehensive understanding of available community resources, assisting youth and families in accessing necessary supports.

• Performs thorough CANS assessments and facilitates the creation of a personalized, strength-based Child and Family-Centered Care Plan.

• Continuously monitors the care plan to ensure services are delivered as outlined, addressing challenges and barriers, and updating the plan as necessary.

• Organizes Child and Family Team meetings, ensuring that children and families have a voice and choices regarding their plans and providers.

• Promotes innovative solutions and the utilization of natural supports, with the aim of keeping youth engaged in their community and in the least restrictive setting.

• Facilitates access to clinical and psychiatric consultations for the assigned ICC and/or MCC youth.

• Develops and reviews safety-crisis plans with youth and families.

• Ensures that initial and ongoing engagement, contacts, and documentation are completed as mandated by CME regulations.

• Flexibility in working hours is essential, which may include evenings and/or weekends, and services will be provided in homes and other community settings.


⛳️ Requirements

• A minimum of 3 years of experience (or 2 years with an Associate or Bachelor’s degree; or 1 year with a Master’s degree) in delivering community-based services and supports to children, youth, and their families or caregivers in fields such as children’s behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or related public sector human services or behavioral health care.

• Background and experience in one or more areas including family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.

• Demonstrates cultural competence or responsiveness with the necessary training and experience to manage complex cases effectively.

• Possesses the qualifications and experience to work with children and families facing SED, trauma, co-occurring behavioral health disorders, and those involved with one or more child-service systems.

• Completion of the high-fidelity wraparound training program provided by an independent validation entity recognized by the ODM (the COE) is required.

• Ability to communicate clearly, both verbally and in writing, with diverse audiences, and willingness to represent the organization within the medical/health-care community.

• Proficiency and accuracy in computer usage is mandatory.

• A valid Driver’s license, acceptable driving record, and insurability through Harbor’s vehicle insurance carrier are required.

• CPR/First Aid and NCI/CPI Certifications must be obtained within 90 days of employment.

• Preference for staff to live in the community they are serving.

• Minimum Internet Service Requirements: Upload speed of at least 5 mbps; download speed of at least 10 mbps per user of dedicated Internet. Internet service providers must be cable or fiber; DSL, satellite, and cellular providers are not acceptable. Business class is preferred.
