Clinical Care Coordinator – Russian, Vietnamese, Chinese Speaking

This is a fully remote position, open to applicants in United States.

📋 Description

• Operate autonomously and oversee a monthly assignment of patients, ensuring comprehensive care for all individuals assigned to you.

• Enhance HealthXL's CCM/RPM platform to maintain consistent and compliant service delivery.

• Train to proficiently navigate various screens (CCM/RPM platform, relevant EMR, 8x8, etc.) and accurately document all findings each month.

• Make numerous attempts to contact each patient monthly and create/update their personalized Care Plan.

• Establish a trusting rapport with patients through regular, compassionate care.

• Manage the communication of essential information to share with the practice on behalf of patients.

⛳️ Requirements

• Reside in the United States.

• Possess clinical credentials such as MA, LPN, RN, BSN, or an other approved qualified background.

• Hold a multi-state compact nursing license (if LPN, RN, BSN).

• Fluent in Russian/Ukrainian, Chinese, or Vietnamese.

• At least 1 year of experience in a clinical or patient-focused environment.

• Exceptional verbal communication skills, demonstrating empathy.

• Knowledge and application of medical terminology as needed.

• Comfortable engaging with and assessing patients remotely; an active listener.

• Ability to identify condition-related warning signs.

• Organized with thorough documentation abilities.
