
Client Success Manager – Value Based Care Platform

This is a fully remote position, open to applicants in Pennsylvania.
• Act as the main point of contact on a daily basis for a designated portfolio of Constyn clients.
• Oversee account health and escalate any emerging risks to the Director of Client Success.
• Generate outcome data, adoption metrics, and account narratives to facilitate Quarterly Business Reviews.
• Manage the onboarding process for new clients to Constyn, coordinating implementation timelines with the Product and Technology teams.
• Provide training and continuous support on platform features, workflows, and reporting tools.
• Monitor adoption and utilization metrics, and create action plans for accounts with low usage.
• Convert platform usage data into client-facing performance narratives linked to quality, population health, and value-based care objectives.
• Collaborate with clients to establish and monitor success plans and key performance indicators (KPIs).
• Address and resolve client platform issues, working alongside Technology and Support teams to ensure prompt resolution.
• Contribute to and apply standardized onboarding, adoption, and success playbooks across the client portfolio.
• Keep CRM and account documentation updated to assist in forecasting and maintaining continuity.
• Bachelor's Degree in Healthcare Management, Business Administration, Public Health, or a related field.
• A Master's degree (MBA, MHA, MPH) is preferred, or additional relevant experience may be accepted in place of an advanced degree.
• 5-7 years of progressively responsible experience in client success, account management, or client-facing delivery roles within healthcare SaaS, health tech, or healthcare operations.
• Experience in supporting value-based care, Medicaid, quality improvement, or population health programs is preferred.
• Proven capability to manage a portfolio of client accounts and collaborate cross-functionally with Product and Technology teams.
• Excellent written and verbal communication skills, including expertise in client-facing training and reporting.
• Analytical approach with the capacity to interpret platform usage data into client-ready insights.
• Proficient in Word, Excel, PowerPoint, Teams, and SharePoint.
• Familiarity with CRM tools and Smartsheet is preferred.
