Chronic Care Management LPN/LVN, Virginia License

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Conduct scheduled outreach for chronic care and perform patient check-ins.

• Reinforce established care plans, treatment objectives, and self-management education.

• Monitor Remote Patient Monitoring (RPM) data and identify trends that necessitate clinical intervention.

• Coordinate follow-up care following hospital discharges or specialty visits.

• Effectively communicate with interdisciplinary care teams.

• Accurately and compliantly document all patient interactions.

• Enhance patient adherence by identifying and resolving barriers.

⛳️ Requirements

• Current Virginia LPN/LVN license in good standing.

• Minimum of 2 years of nursing experience, preferably in chronic care or telehealth.

• Familiarity with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or value-based care models is advantageous.

• Strong skills in clinical assessment and patient communication.

• Ability to work independently in a remote clinical setting.
