This is a fully remote position, open to applicants in Maryland.
• Conduct timely evaluations of patient medical records to ensure precise and comprehensive documentation that accurately represents the severity and complexity of the patient's illness.
• Collaborate with healthcare providers and coders to guarantee that documentation reflects the intricacy of each patient case.
• Review assigned medical records for thorough and accurate documentation of all pertinent diagnoses and procedures.
• Initiate and complete queries as necessary to ensure data accuracy.
• Comply with guidelines established by ACDIS, AAPC, and AHIMA.
• Uphold strict confidentiality of all patient information in line with HIPAA regulations.
• Demonstrate a solid understanding of the healthcare business, including DRG payer issues, audit risks, documentation opportunities, quality metrics, and hierarchical condition categories.
• Collaborate closely with clients, colleagues, and management to address the specific needs of each assignment while aligning with core values.
• Effectively leverage computer software in accordance with specific client needs.
• Minimum of 2 years of clinical documentation experience.
• Active Registered Nurse (RN) license.
• Preferred experience with Maryland Facility APR-DRG.
• Familiarity with Epic and 3M 360.
• Manage 6-10 cases per day while maintaining a 30% query rate.
• Proficient in using a PC for medical record reviews and skilled in Microsoft products (Excel, Word, Outlook).
• Strong communication skills to effectively interact with e4health management and clients.
