Casemanager, Dementie

This is a fully remote position, open to applicants in Netherlands.

📋 Description

• Identifying and analyzing the care needs of home-dwelling clients with (suspected) dementia and their caregivers.

• Ensuring cohesive care and providing support to both the client and caregiver.

• Visiting clients at home and documenting appointments in the Electronic Client File (ECD).

• Developing care plans.

• Responding swiftly and effectively in crisis situations.

• Monitoring the needs and preferences of the client and caregiver.

⛳️ Requirements

• HBO diploma in Nursing (level 5/6 and BIG registered) or a completed HBO diploma in SPH/Social Work with additional post-HBO case management in dementia.

• Affinity for and experience with the dementia target group.

• Independent, enjoys challenges, entrepreneurial, takes initiative, and can effectively pioneer.

• Extensive network and able to establish contacts easily.

• Significant experience with the Social Map is preferred.
