Case Manager, Registered Nurse - Field

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Develops a proactive care plan to address identified concerns.

• Utilizes clinical tools and reviews data to evaluate members' needs and benefits.

• Employs clinical judgment to formulate strategies that minimize risk factors and obstacles.

• Conducts assessments by integrating information from diverse sources.

• Adopts a holistic perspective to determine the necessity for referrals to clinical resources.

• Collaborates with healthcare supervisors and relevant stakeholders.

• Implements case management processes in accordance with regulatory and company policies.

• Applies motivational interviewing techniques to enhance member engagement.

• Engages with members or clients via telephone or in-person interactions.

• May conduct meetings with members or clients in various environments for ongoing case management support.

⛳️ Requirements

• Must be a resident of the state of Illinois.

• Should have reliable transportation and be willing to travel up to 75% of the time from home.

• A minimum of 3-5 years of clinical practical experience is required.

• Confidence in working from home as an independent thinker, utilizing tools to virtually collaborate and connect with teams.

• Strong analytical and problem-solving abilities.

• Excellent communication, organizational, and interpersonal skills.

• Capability to work independently.

• Proficient computer skills, including the ability to navigate multiple systems and keyboard effectively.

• Demonstrates competence with standard corporate software applications such as MS Word, Excel, Outlook, and PowerPoint, along with some proprietary applications.

• 2-3 years of experience in Care Management, discharge planning, and/or home health care coordination.

• Certified Case Manager designation.

• Bilingual proficiency is preferred.
