Case Manager, Registered Nurse

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Evaluate patient requirements and implement care coordination strategies.

• Create tailored care plans and ensure smooth transitions between different care environments.

• Review patient information, including medical histories, diagnostic results, and treatment plans.

• Utilize case management and nursing expertise to organize patient documentation.

• Communicate securely via internal platforms such as messaging systems and video calls.

• Perform utilization assessments to guarantee access to suitable and cost-effective healthcare.

• Set up case management systems focused on disease management and utilization evaluation.

• Monitor patient advancement and manage specific health conditions.

• Coordinate analytics initiatives and prepare reports on essential health performance metrics.

• Develop discharge planning procedures to facilitate safe and prompt transitions from hospital to home.

• Formulate resource management plans using community healthcare resources.

• Represent patients and coordinate services among various healthcare providers.

⛳️ Requirements

• Over 5 years of pertinent work experience.

• Currently active and unrestricted Registered Nurse License in the state of residence.

• Proficient in problem-solving and decision-making capabilities.

• Familiarity with medical terminology.

• Competence in digital literacy skills.

• Ability to engage tactfully with clients and the community.

• Capability to manage sensitive information in an ethical and responsible manner.

• Aptitude for evaluating the relative costs and benefits of possible actions.

• Ability to operate effectively in a clinical environment with diverse cultural dynamics.

• Direct/hardwired internet connection to a modem/router within 7 feet of the computer.

• Minimum internet speed of 25 Mbps for downloads and 3 Mbps for uploads.

• WiFi and satellite internet connections are not acceptable.

• A quiet, secure, and private designated home workspace that complies with CVS Health and HIPAA standards.

• Must reside and work within the same state and city.

• Associate’s Degree in Nursing (mandatory).

• Bachelor’s Degree (preferable).
