Case Manager – Registered Nurse

This is a fully remote position, open to applicants in Texas.

📋 Description

• Serves as a point of contact for members/clients/families, employers, providers, insurance companies, and healthcare professionals.

• Oversees and coordinates all case management tasks related to catastrophic cases and chronically ill members/clients throughout their care journey.

• Engages with members/clients either by phone or in person.

• Evaluates and assesses the medical and/or vocational status of injured, acute, or chronically ill members/clients.

• Creates a care plan to aid in the effective management of the member/client’s condition.

• Communicates with members/clients and other involved parties.

• Prepares all necessary documentation for case management activities.

• Collaborates and consults with an internal multidisciplinary team.

• Offers educational resources and preventative information to ensure optimal medical outcomes.

• Applies case management processes in accordance with regulatory requirements and company policies and procedures.

• Tracks and monitors member/client progress towards achieving desired outcomes.

⛳️ Requirements

• Candidates must reside in a state within the Eastern Standard Time Zone (EST).

• Candidates must possess an active and unrestricted Compact Registered Nurse (RN) license in their state of residence.

• A minimum of 2 years experience in care management is required.

• At least 2 years of experience with Medicare is necessary.

• Experience in geriatric care is preferred.

• Experience in a multicultural environment or personal multicultural experience is preferred.

• Certification as a Case Manager through a national professional organization (such as CRC, CDMS, CRRN, COHN, or CCM) is preferred.

• An Associate’s Degree in Nursing along with relevant experience in a healthcare-related field is REQUIRED.

• A Bachelor’s Degree in Nursing is PREFERRED.
