• Engage in a collaborative approach involving assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to address the comprehensive health needs of individuals and families.
• Serve as a liaison among members/clients/families, employers, providers, insurance companies, and healthcare personnel as necessary.
• Implement and oversee all case management activities related to catastrophic cases and chronically ill members/clients throughout the continuum of care.
• Communicate with members/clients either via phone or in person.
• Evaluate and analyze the medical and/or vocational status of injured, acute, or chronically ill members/clients.
• Create a care plan to support the effective management of the member/client's condition.
• Maintain communication with the member/client and relevant stakeholders as needed.
• Complete all necessary documentation regarding case work activities as appropriate.
• Candidates must possess an active and unrestricted Compact Registered Nurse (RN) Licensure.
• A minimum of 3 years of clinical practical experience is preferred in areas such as diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, or Home Health involving Medicare members.
• At least 2 years of experience in case management, discharge planning, and/or home health care coordination is required.
• Proficiency in standard corporate software applications, including Microsoft Word, Excel, Outlook, and PowerPoint, along with some specialized proprietary applications.
• Ability to thrive in a fast-paced, high-volume environment while effectively managing time and prioritizing tasks.
• Strong computer skills, including the ability to navigate multiple systems and proficient keyboarding capabilities.
