Case Manager, Registered Nurse

This is a fully remote position, open to applicants in New York.

📋 Description

• Allocate 50-75% of the day to telephonic interactions with members and coordinating their care.

• Gather all pertinent clinical information and work alongside the member to create a personalized care plan.

• Offer evidence-based education on disease management and support to assist the member in reaching their health objectives.

• Ensure that the relevant members of the interdisciplinary care team are engaged in the member’s care.

• Facilitate care coordination to provide a smooth healthcare experience for the member.

• Diligently document care management activities within the member’s electronic health record.

• Collaborate with other members of the Interdisciplinary Care Team.

⛳️ Requirements

• Must possess active and unrestricted Registered Nurse (RN) licensure in the state of NY.

• Skilled in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams.

• Access to a private and dedicated workspace to perform work effectively.

• A minimum of 3+ years of nursing experience is required.

• At least 2+ years of experience in case management, discharge planning, and/or home healthcare coordination is needed.
