• Oversees the care coordination plan to evaluate patient requirements and guarantee smooth transitions across various care environments.

• Evaluates intricate patient data from medical histories, diagnostic test outcomes, and treatment plans to ascertain the current health condition of the patient.

• Utilizes comprehensive knowledge of case management and nursing methodologies to arrange patient records.

• Engages via internal communication platforms to safely exchange messages, hold video meetings, and share documents.

• Performs regular utilization assessments to confirm access to suitable and cost-effective healthcare.

• Sets up the case management system to organize cases related to disease management and monitor patient advancements.

• Coordinates analytical projects to assess data and create reports on health metrics.

• Develops intricate processes to manage discharge planning from the hospital to home settings.

• Establishes resource management strategies to enhance healthcare using community resources.

⛳️ Requirements

• Must possess an active and unrestricted Illinois RN license.

• 3-5+ years of clinical experience is required.

• Registered Nurse (RN) certification is mandatory.

• 1-3+ years of experience in Case Management is essential.

• Experience in Telephonic Nursing is required.

• Background in Home Health is preferred.

• Certification as a Case Manager is an advantage.

• An Associate's Degree is required.

• A Bachelor's degree is preferred.
