Case Manager, Bilingual

This is a fully remote position, open to applicants in Philippines.

📋 Description

• Act as the main point of contact between patients and caregivers

• Oversee and ensure the quality of care by fulfilling VBP (Value-Based Purchasing) criteria

• Address escalated non-clinical concerns from patients and caregivers

• Keep precise and current records for all caregivers and patients

• Collaborate closely with the patient care coordinator regarding scheduling and operational matters

⛳️ Requirements

• Registered Nurse with a valid NY License - USRN NY

• Bilingual in Spanish is a requirement

• At least 3 years of experience in case management or a related field

• Exceptional verbal and written communication abilities

• Strong interpersonal skills and a focus on customer service

• Capability to manage challenging or sensitive situations with professionalism

• Familiarity with HHA Exchange is highly desirable
