Case Manager

This is a fully remote position, open to applicants in Arizona, +9 more states.

📋 Description

• Evaluate the needs of members in various domains including physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social support, housing, and connections to community-based services.

• Supervise the creation of client care plans and the establishment of goals.

• Provide services at locations convenient for the member, whether that be in-person at an office, through telehealth, or in the field.

• Link clients to necessary social services and supports.

• Act as an advocate for clients with healthcare professionals, such as primary care physicians (PCP).

• Employ evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care methodologies.

• Conduct outreach and engagement activities to promote connection to the Enhanced Care Management (ECM) program and record activities in the Client Relationship Management (CRM) system.

• Assess client progress and revise SMART goals accordingly.

• Facilitate mental health promotion initiatives.

• Organize transportation arrangements, such as ACCESS.

• Complete all necessary documentation, including outcome measures, within the timelines outlined in individual care plans.

• Keep patient health records current in the Electronic Medical Record (EMR) system and other relevant business systems.

• Submit monthly reports to ensure compliance with program standards.

• Participate in assigned training sessions.

⛳️ Requirements

• A minimum of 2 years of experience as a care manager, care navigator, or community health worker supporting at-risk populations.

• Availability to work Monday to Friday from 8:30 am to 5:00 pm Pacific Time, with potential flexibility for evening and weekend shifts.

• Familiarity with government and community resources pertaining to social determinants of health.

• A clean driving record, a valid driver's license, and dependable transportation.

• Strong oral and written communication abilities.

• Positive interpersonal skills are essential.

• Proficient computer skills along with a working knowledge of Google Workspace, Microsoft Office, and the internet.

• Bilingual proficiency in English and Spanish is preferred.
