This is a fully remote position, open to applicants in Arizona, +9 more states.
β’ Evaluate the needs of members in various domains including physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social support, housing, and connections to community-based services.
β’ Supervise the creation of client care plans and the establishment of goals.
β’ Provide services at locations convenient for the member, whether that be in-person at an office, through telehealth, or in the field.
β’ Link clients to necessary social services and supports.
β’ Act as an advocate for clients with healthcare professionals, such as primary care physicians (PCP).
β’ Employ evidence-based practices including Motivational Interviewing, Harm Reduction, and Trauma-Informed Care methodologies.
β’ Conduct outreach and engagement activities to promote connection to the Enhanced Care Management (ECM) program and record activities in the Client Relationship Management (CRM) system.
β’ Assess client progress and revise SMART goals accordingly.
β’ Facilitate mental health promotion initiatives.
β’ Organize transportation arrangements, such as ACCESS.
β’ Complete all necessary documentation, including outcome measures, within the timelines outlined in individual care plans.
β’ Keep patient health records current in the Electronic Medical Record (EMR) system and other relevant business systems.
β’ Submit monthly reports to ensure compliance with program standards.
β’ Participate in assigned training sessions.
β’ A minimum of 2 years of experience as a care manager, care navigator, or community health worker supporting at-risk populations.
β’ Availability to work Monday to Friday from 8:30 am to 5:00 pm Pacific Time, with potential flexibility for evening and weekend shifts.
β’ Familiarity with government and community resources pertaining to social determinants of health.
β’ A clean driving record, a valid driver's license, and dependable transportation.
β’ Strong oral and written communication abilities.
β’ Positive interpersonal skills are essential.
β’ Proficient computer skills along with a working knowledge of Google Workspace, Microsoft Office, and the internet.
β’ Bilingual proficiency in English and Spanish is preferred.
