Case Manager

This is a fully remote position, open to applicants in United States.

📋 Description

• Develop and promote adherence to treatment plans established by patients’ healthcare providers.

• Create and maintain precise, detailed clinical patient reports for clients and insurance carriers.

• Monitor and document adherence to treatment plans, assess effectiveness, and track patient progress in a timely manner.

• Enhance continuity of care by managing relationships with tertiary care providers, transitions-in-care, and referrals.

• Improve patients’ capacity for self-management and shared decision-making.

• Foster understanding through culturally and linguistically appropriate educational materials.

• Assist with medication reconciliation processes.

• Connect patients to pertinent community resources to enhance their health and well-being.

• Act as the primary contact, advocate, and information resource for patients, care teams, family/caregiver(s), stop-loss carriers, and community resources.

• Assess patients’ unmet health and social needs.

• Uphold patients’ rights regarding confidentiality, consistently maintaining confidentiality and adhering to HIPAA guidelines and regulations.

⛳️ Requirements

• Must hold current registration as a registered nurse (RN) or licensed practical nurse (LPN).

• A minimum of 5 years of experience in a clinical or community resource setting or care coordination is required.

• At least 2 years of case management experience is mandatory.

• Highly organized with the ability to maintain accurate notes and records.

• Demonstrated essential leadership, communication, education, and counseling skills.

• Proficient computer skills and the capacity to learn new systems effectively.

• Exceptional telecommunication skills are required.

• Maintains a high level of professionalism and confidentiality at all times.
