Case Manager

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Engaging with individuals on the caseload and participating in team meetings.

• Conducting or attending meetings for needs assessments and the development of individual service plans (CSSP).

• Aiding in the planning of new service initiatives.

• Identifying residential, vocational, and additional essential services tailored to each individual's needs.

• Overseeing service delivery to ensure the health and safety requirements of each individual are fulfilled.

• Completing referrals and providing crisis intervention as necessary.

• Formulating waiver budgets.

• Documenting case notes.

• Serving as an advocate for the individuals we support.

⛳️ Requirements

• Must be a licensed Social Worker (Bachelor of Social Work required for licensure).

• Alternatively, be a licensed Graduate Social Worker (Master’s in Social Work required for licensure) as specified by the Minnesota Board of Social Work.

• Or possess a 4-year degree in Human Services, Psychology, Sociology, or related fields.

• Successful completion of the Department of Human Services background check.

• Successful completion of the Motor Vehicle Background Check and maintain an acceptable driving record in accordance with Company Policy.
